The symptom screener can help you find out if the changes you're noticing could be symptoms of Alzheimer's.

Important: This screening tool cannot be used to tell if your loved one has a medical problem, only whether he or she should be tested. Remember, only a doctor can diagnose Alzheimer's or any type of dementia.* This tool is not intended to depict information about ARICEPT.

Answer the eight questions below about changes you have noticed and share them with the doctor.

Remember, "Yes, a change" indicates that there has been a change in the last several years caused by cognitive problems.


  Problems with judgment (problems making decisions, bad financial decisions, problems with thinking, etc.)
  Less interest in hobbies/activities
  Repeats the same things over and over (questions, stories, or statements)
  Trouble learning how to use a tool, appliance, or gadget (e.g., VCR, computer, microwave, remote control)
  Forgets correct month or year
  Trouble handling complicated financial affairs (balancing checkbook, income taxes, paying bills, etc.)
  Trouble remembering appointments
  Daily problems with thinking or memory
